Does FORME replace my EMR?

No. FORME is not an EMR and never acts as the record of care. Your EMR stays the source of truth.

FORME is not an electronic medical record and does not try to be one. It uses information from documents and devices to give the coach and the plan context, but it is never the system of record.

Uploaded documents are read, the relevant facts are extracted, and the original file is deleted. The authoritative copy stays where it was created: your EMR, or the patient's own records.
