What can I expect when I bring FORME into my practice?

Fewer calls to your front desk. A handout can't answer questions, so patients call. FORME holds the instructions your practitioners and clinic provide (test preparation, prescription directions, appointment details and care instructions) and patients can ask about them any time: "Do I need to fast before my blood test?", "Can I take this with food?", "What should I bring to my appointment?" Answers come from your clinic's own instructions. For prescriptions, the coach repeats your directions faithfully and doesn't go beyond them; anything outside your instructions goes back to your team. More satisfied patients. Every patient gets a health coach of their own, available 24/7, that knows their plan, their results and their progress. Patients feel looked after between visits, not just during them. Better adherence and better outcomes. Daily goals, check-ins and a coach that adapts to how they slept and moved keep patients working on the plan your practitioners approved. What does FORME do with patients' wearable data?

A wearable on its own reports numbers: sleep, heart rate variability, resting heart rate, activity. A clinic on its own sees the patient a few times a year. FORME brings the two together, and that combination produces insight that isn't otherwise available. Wearable trends are read against the patient's own baseline and against age- and sex-based reference ranges, not just a device score. They are interpreted in light of what the clinic knows: lab results, medications and dose changes, the approved plan, care instructions and your clinic's protocol rules. In practice that means the coach can connect a habit to a lab result (for example, consistent cardio alongside an improved LDL), ease a patient's plan on days their body data says they need it, apply a clinic rule such as an easier training week after a medication dose increase, and credit goals automatically when a workout is logged. Your team sees the same picture in the practitioner portal, so the next visit starts from what actually happened between visits, not from memory. Wearable readings are used for coaching and context, never as a diagnosis.

How much does FORME cost for a clinic?

FORME is one plan: $349 CAD a month. It includes 30 active patients and as many practitioners and staff as your clinic needs. Each patient beyond 30 is $5 a month. A patient counts as active from their first sign-in until you remove them. Remove a patient and they stop counting from the next billing cycle. How does FORME save a clinic money?

To give patients more comprehensive care, and better care that actually gets them healthy, many clinics extend their practice by hiring a nutritionist or an exercise trainer. That person becomes the patient's health coach, guiding them on nutrition, movement and daily habits between visits. Adding one of those roles typically costs a clinic $4,000 or more a month. FORME extends your practice the same way. Every patient gets a daily AI health coach that works from your clinic's protocols and your practitioners' plan, under your team's supervision. Your patients get more comprehensive care without you adding headcount. FORME is not a replacement for clinical judgment. Your practitioners approve the plan and stay in the loop; FORME carries it out day to day.

Question 5

Can FORME follow my clinic's own protocols?

FORME comes preloaded with guideline-based longevity defaults (for example, movement programs based on ACSM, AHA/ACC and CCS guidance). Your clinic's protocols override those defaults wherever they overlap. To add a protocol, you upload the document. FORME extracts its actionable rules in a simple form: when this happens, do this, and tell this person. A clinician on your team approves, edits or rejects each rule. Only approved rules take effect, and they are applied by a rules engine, not improvised by the AI at the time. That keeps clinical decisions with your clinicians.

Who checks what the AI recommends to patients?

When a patient joins, FORME drafts a plan from their intake, documents and wearable data. The plan stays hidden from the patient until a practitioner at your clinic reviews, edits and approves it. When a practitioner corrects something FORME suggested, that correction is saved and becomes a rule the coach follows for your clinic from then on. Safety checks run alongside the coach: messages that suggest a crisis switch the coach into safety mode, show crisis resources and alert your practitioners, and medical values that are implausible are flagged instead of being used.

Does FORME replace my EMR?

FORME is not an electronic medical record and does not try to be one. It uses information from documents and devices to give the coach and the plan context, but it is never the system of record. Uploaded documents are read, the relevant facts are extracted, and the original file is deleted. The authoritative copy stays where it was created: your EMR, or the patient's own records.

How do my patients get started?

Each clinic gets its own invite link. Patients who join through it are linked to your clinic automatically. They complete a guided intake, can upload lab reports and other documents, and can connect wearables such as Oura, Whoop, Apple Health, Garmin or Fitbit. Their plan goes to your team for approval before they see it. FORME is available on the web, iPhone and Android.

Who owns my health data?

Accepted Answer

Your health data belongs to you, not to FORME and not to your clinic. FORME uses it to coach you and to give your care team the context they need to look after you. It is never sold or shared for advertising. You stay in control of all of it. From Settings you can download a copy of your data at any time, and you can delete your account yourself. When you delete your account, all of your data is completely and permanently deleted, and your wearables are disconnected. Uploaded documents are never kept in the first place: they are read, the relevant results extracted, and the original file permanently deleted.

What happens to the medical records I upload?

When you upload a lab report or other document, FORME reads it, extracts the results that matter for your plan, and then permanently deletes the original file. We keep the information, not the document. Your data is encrypted in transit and at rest, and it is never sold. The data is yours. You can download a copy, or delete your account yourself at any time from Settings, which completely deletes all of your data.

Who can see my health data?

Your information is visible only to you and to the care team at the clinic you joined through FORME. Access is enforced in the database itself, not just in the app. It's your data and you stay in control of it: you choose the clinic you join, you can disconnect your wearables at any time, and you can download or delete your data from Settings. Which wearables and apps work with FORME?

FORME connects to Oura, Whoop, Apple Health, Garmin, Fitbit, Eight Sleep and Android Health Connect, along with other devices supported through our wearable data partner. Your wearable data isn't read on its own. FORME combines it with what your clinic knows (your lab results, medications, approved plan and care instructions) to give you insight your device can't: how your habits connect to your results, and what your body data means for today's plan. Sleep, heart rate variability, resting heart rate, activity and other measures are compared with your own baseline and with reference ranges for your age and sex, so the coaching adjusts to how you actually slept and moved. No wearable? FORME still works from your intake, lab results and daily check-ins.

Is FORME medical advice?

FORME helps you understand your data and follow the plan your clinic approved. It does not diagnose conditions, prescribe treatment, or replace your practitioner. The coach won't advise on starting, stopping or changing a medication or supplement. It refers you to the clinician who prescribed it and can flag the question to your practitioner.

My clinic doesn't use FORME yet. What can I do?
